By accessing our website or booking an appointment, you agree to be bound by these terms.

1. Services Provided

A Truly Unique Touch CPR and Diagnostics provides mobile specimen collection, wellness screenings, CPR and safety certification, and chronic condition coaching.

• Medical Orders: All diagnostic and phlebotomy services require a valid lab order from a licensed healthcare provider prior to the appointment. We do not order tests or interpret lab results.
• Service Area: We provide mobile services to locations within our designated service area. Appointments requested outside this area may be subject to additional travel fees or cancellation.

2. Payment, Cancellation, and Rescheduling Policy

To ensure we can reliably serve all our clients, we strictly enforce the following policies regarding payments and appointments:

• No Refunds: All payments for services, training sessions, and mobile visits are final. We do not offer refunds under any circumstances.
• Rescheduling: If you cannot make your scheduled appointment, you may transfer your payment to a new appointment date a maximum of two (2) times.
• Forfeiture of Payment: If you fail to attend your appointment after the second rescheduled date, no further reschedules will be permitted. You will forfeit your original payment, and any future visits will require a new booking and full payment of the service fee.
• Extreme Circumstances: We understand that true emergencies happen. If an extreme situation prevents you from keeping your appointment, please contact our office directly prior to your scheduled visit. Management will review these situations on a case-by-case basis.

3. Client Responsibilities

• Accuracy of Information: You agree to provide accurate, current, and complete information during the booking and intake process, including medical history and physician details where applicable.
• Environment Setup: For mobile and in-home visits, you agree to provide a safe, clean, and well-lit environment for our staff to perform the requested services.
• Pets and Safety: For the safety of our staff and the integrity of the clinical procedure, all pets must be secured in a separate room prior to the arrival of our phlebotomists or educators.
